Office of the Executive Vice President and Provost; Office of the Chief Human Resources Officer
(President 11/9/78; amended 8/99; 4/01; 6/01; 1/17; 4/17; 12/9/24; 8/21/26)
Effective August 21, 2026, this policy has been revised. For individual changes, see the redlined version.
- The university occasionally uses administrative surveys and questionnaires undertaken by persons acting in an administrative capacity or under the auspices of the university administration to gather information from respondents across multiple colleges or administrative units. These surveys assist the institution in conducting its operations and may inform university-wide strategic planning or provide responsive information to accrediting agencies.
- To ensure that this information is gathered in an accountable and open manner, the university adheres to a regular and consistent approval procedure for administrative surveys and questionnaires that 1) are directed to respondents in more than one college or administrative unit, and 2) are not related to research subject to IRB review (see II-27.4, II-27.6). This policy does not apply to surveys by a college, department, or program that are directed solely to its own students, faculty, or staff for internal planning or operational purposes.
- Approval of administrative surveys and questionnaires must be secured from the Administrative Survey Panel. This panel shall be composed of the designees for the offices of the Executive Vice President and Provost, the Vice President for External Relations, the Chief Human Resources Officer, the Division of Student Life, and the Office of Assessment.
- In addition to confirming that all administrative surveys and questionnaires are designed and implemented in a fashion that respects the rights of individuals on campus, this procedure provides a comprehensive view of the many university-wide information-gathering activities of the university and a means for systematic administrative review of such activities.
